Student Health Information Requirements

  • Student Health Information & Immunization Records: All incoming students must complete their Health History, TB Questionnaire, and verify their immunizations through the Student Health Portal. The Student Health Portal connects with Pennsylvania’s state immunization registry to automatically import in-state vaccination records, while the VeriVax tool retrieves out-of-state records through a national network for easy verification.
    • Click on the Immunization tab for a list of required vaccinations and instructions on verifying your immunization compliance.
    • Click on the Forms tab to complete your Health History and TB Questionnaire.

Recommended Information: Uploading the following to your Student Health Portal is optional, but can help us better support your care. 

  • A copy of your insurance card. 
  • A copy of your most recent physical exam within 1 year of the start of the current semester. You may use our Health Evaluation Form or submit paperwork provided by your physician.

  • Please note: Submitting a Health Evaluation/Physical exam is recommended but optional.

  •  Note:  Records must be uploaded electronically to the Student Health Portal. Student Health Services will not accept faxed or emailed records. All documents must be legible and in English.   

Follow these steps to upload documents:

  1. Log into your .
  2. Click “Upload” from the menu bar at the top of the page.
  3. You can choose which document to upload from the drop-down list, such as a freshman physical exam, immunization history, insurance card, or meningitis waiver.
  4. Click the Select file to see all your electronic files on your computer or smartphone.
  5. Click on the electronic file you want to upload.
  6. Click Upload.
